This is an exciting opportunity to join Team Caerphilly and provide support across the wider Physical Disability and Sensory Impairment / Learning Disabilities Teams.

Diploma/Degree in Social Work or predecessor qualification and be registered as a Social Worker with Social Care Wales (or able to transfer from another recognised body).
Knowledge of the social care legislative framework
An understanding of the Learning, physical and emotional needs of Vulnerable Adults with Physical disabilities and sensory impairment/ Learning disabilities
effective communication and organisational skills to support and safeguard vulnerable service users through difficult periods in their lives, Disability Confident

We pay an attractive salary of £36,648 - £39,186 and offer access to training and development opportunities.

The successful candidate will be joining a dynamic and forward thinking multi disciplinary team who work with adults with a Learning or Physical Disability and/or sensory impairment, embracing an outcomes focused approach.

The team operates in a multi-agency manner, working with colleagues from, health, housing and other statutory services, in addition to internal services, including our Direct Payment Service; Children's services, to support young people through transition to adulthood, and our dedicated Rehabilitation Officers for the visually impaired.

The team works in accordance with National Occupational Standards for Social Work and Code of Practice for Social Care Workers, and offer balanced and varied Caseloads, including the opportunity for fast-paced hospital discharges, contrasted with longer term pieces of work, developing professional relationships with service users and making use of learnt knowledge about specific conditions and disabilities.

The team are extremely supportive, creating a learning environment through regular supervision, appraisal, and peer reflective sessions. The department are invested in the C-Pel programme and ensure a good range of relevant training courses are available to staff.
