Full Job Description

Job Purpose To provide community nursing care to a wide variety of patients in their own home. To work within the wider integrated health and social team in the community, providing holistic care to patients. This nursing care includes acute and chronic wound management, medication administration, catheter care, palliative care, long term conditions/ self-care. To provide holistic and individualised care to patients and carers as delegated by the Team Leader or deputy, working autonomously and as part of a team. To regularly take responsibility for care and workload management in the absence of the Team Leader or deputy. Dimensions To provide nursing care across a diverse population for patients who are registered with a Nottingham City GP To participate actively in the services training strategy, including mandatory training and other identified necessary for this role To work across the Nottingham City Community Nursing Teams To supervise and support student nurses on placements with
the community nursing teams. To supervise unregistered nurses who have been delegated patient care. Key Responsibilities Contribute to the public health agenda by raising awareness of health and well-being needs to individuals. Plan, deliver and evaluate programmes of care to address peoples health and well-being needs. Implement the provision of evidence-based/ best practice. Actively encourages patient and carer feedback about the service Maintain a high standard of care to patients and carers. Contribute to the continual improvement of the service. Maintain a professional image as outlined in NMC code of conduct. Contribute to the Clinical Governance agenda and applies risk management systems Participate in relevant research and audit activity Regularly takes responsibility for the appropriate delegation of nursing care to team members. Promote and maintain good practice in record keeping by self and the team to ensure NCCP and Professional standards are met. Establish and maintain
communication with individuals and groups in a wide variety of settings and situations in the Primary Health Care Team. Liaise and interact with other agencies and disciplines To be responsible for the communication and effective feedback to the team leader or deputy Support effective team working both in the team and the Primary Health Care Team To be responsible for the development of own knowledge and clinical practice and recognises ones own limitations. Act as a mentor to a variety of nursing students Participate in the development of other team members. Participate in the supervision framework as a supervisee and supervisor. Maintain a portfolio to demonstrate continuing professional development Contribute to the implementation of service developments. Participate in the Personal Development process and take responsibility for reaching agreed development objectives Attend mandatory training Utilise enhanced knowledge and skills to promote continuous
service improvements Promote and maintain best practice that ensures the health safety and security of self, patients and colleagues in accordance with the policies procedures and guidelines of NCCP. Ensure that confidentiality is protected at all times Practice in accordance with procedures and guidelines for the Protection of Vulnerable Adults and Child Protection. In the absence of District Nurse Team Leader or deputy takes responsibility for the needs of patients and carers on the community nursing caseload and ensures that appropriate delegation and supervision of work is carried out in accordance to the skills and abilities of team
