Full Job Description

Holistic assessment of and intervention for patients experiencing or at risk of frailty and falls in their own homes, GP surgeries and care homes. Ensure all assessments and interventions are patient-centred, clinically effective and of sound evidence-base. work with patients through a shared-decision making approach to plan realistic, outcomes-focused goals Ability to manage a caseload, work independently as well as part of a multi-disciplinary team to provide specialist, evidence-based, holistic assessment and intervention to patients. Adopt a proactive, early intervention approach to patient care, to reduce reliance on secondary care. formulate and apply relevant occupational analysis to the individual, their activities, and their environment Ensure that all patients that have unplanned contact with urgent care settings have their needs addressed where appropriate. Ability to motivate patients to engage in self-management of their health conditions help patients to adapt to and
manage their physical and mental health long-term conditions, through the teaching of coping strategies Ability to deliver Occupational Therapy intervention on both an individual and group basis to include presenting at pop up events hosted by the PCN Have the knowledge of, sign post and refer on to other professionals and agencies where appropriate, to ensure holistic patient intervention. Possess good communication skills in both verbal and written forms to allow effective MDT working and communication with patients and their carers, considering alternative modes of communication where appropriate. Work collaboratively with social services and other community colleagues and to enhance the patient experience. develop, implement and evaluate a seamless occupational therapy support service across the PCN, working with community and secondary care where appropriate, and aimed at continuously improving standards of patient care and wider MDT working maintain an up to date knowledge of
self-management advice, specialist equipment, environmental adaptation, technology and legislative responsibilities which may support increased function and/or management (for onward referral / request to secondary care) advise patients, and their families or carers, on specialist equipment, adaptations and organisations that can help with daily activities Attend practice/multi-disciplinary/community meetings where required. Contribute to the development of the Occupational Therapy Service within East Lindsey Primary Care Network. develop and design patient and carer information, including advice to support management of conditions where needed provide education and specialist expertise to PCN staff, raising awareness of good practice occupational therapy techniques ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve corporate PCN and local population objectives as directed Apply policies and procedures in line with
the Occupational Therapy Profession and Practices along with those of ELPCN. Support the general practice team to ensure compliance with CQC standards. Support practice level QOF activity where required. Attend training and supervision sessions as required and contribute to in house cpd sessions/shared learning. achieve and demonstrate agreed standards of personal and professional development, actively participate in mentoring and supervision develop and share learning with colleagues, promoting peer review and best practice within the work environment, alert other team members to issues of quality and risk participate in relevant audit processes, evaluation and implementing ongoing service change to order to meet patient need as directed maintain accurate, contemporaneous healthcare records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice to engage in regular
