Full Job Description

An opportunity has arisen to appoint into a key leadership role in the Rhondda Taff Ely Crisis Resolution Home Treatment Team (CRHTT). The post holder will work as a leader and senior clinician within a dynamic and fast paced Crisis Resolution/Home Treatment service as well as provide support, guidance and direction to colleagues with the Community mental Health Teams, and Admissions Unit within the Royal Glamorgan Hospital. They will work with the support of the Medical and leadership team to ensure there is a timely and proportionate response to acute mental health need and crisis within the Rhondda and Taff Ely Locality, and be creative in enabling the CRHTT to provide opportunities for early discharge.,

  • Provide effective leadership for a team of staff providing a high quality service within the resources available within a defined area.

  • Foster robust working relationships with all disciplines and partners.

  • To respond as required to serious and untoward incidents, complaints and communications regarding team issues providing feedback to the team to ensure lessons learnt are shared and inform practice feedback.

  • Work as part of a team, fostering good communication with colleagues of all disciplines whilst ensuring that all communications are conducted with dignity and respect. Manage highly complex and contentious information effectively demonstrating excellent communication skills, both orally and in writing.

  • Promote effective communication within the team by organising and chairing handover meetings, regular team meetings, cascading information, discussing pertinent issues and directing the team where necessary.

  • Be responsible for providing accurate, current, comprehensive and concise records in accordance with the Nursing Midwifery Council Standards for Records and Record Keeping, the Health Board's Patients Record Policy and in compliance with the Data Protection Act.

  • Participate in meetings/represent the Senior Nurse in Directorate/Divisional meetings when requested.

  • Demonstrate good organisational skills and efficient use of resources when preparing a rota, managing annual leave, planning training, deploying staff to support both crisis and home treatment and the Crisis Hub according to competency and workload thus meeting identified patient needs and prioritises.

  • Directly responsible for the recruitment of staff of all disciplines within the CRHT.

  • Responsibility for the maintenance of health and safety standards both within the CRHT base and in the community. This includes the development of both clinical and general risk assessment plan.

  • To ensure delivery of health board policies and procedures such as sickness, lone worker and dignity at work .

  • Undertake investigations and make recommendations on behalf of the health board, including complaints, chronologies and clinical reviews.

  • Manage and maintain a diary of all work activities, making it available for audit as necessary.

  • Take the lead role in the management of complex cases, provide skilful and comprehensive assessment of clients referred to the crisis and home treatment service ensuring risk assessment is an integral component of clinical work and to support staff in such cases ensuring care is delivered and evaluated on a regular basis.

  • Assess and manage critical/ unpredictable situations that arise within the service on a day to day basis.

  • Demonstrate ability to present cases to the wider MDT considering best treatment options and making recommendations to support the patients who is in crisis and or home treatment.

  • Establish rapport and maintain supportive professional relationships with clients and their careers, even when there are barriers to acceptance such as lack of insight or difficulties adjusting to complex life changes; recognising the stress associated with caring.

  • Be responsible and accountable for receiving and providing highly complex, contentious and sensitive information to patients regarding their health status or future treatment.

  • Participate in meetings/represent the Senior Nurse in Directorate/Divisional meetings when requested.

  • Lead and coordinate the work of the multidisciplinary CRHT.

  • Provide leadership to the nursing staff within CRHT.

  • Manage the performance of the team through PDR's, training needs analysis and supervision.

  • Oversee a system of caseload management within the CRHT service.

  • Develop and implement changes in clinical practice both within CRHT and to the wider mental health group.

  • Undertake surveys or audits, as necessary to own work.


  • The post holder will be required to teach other staff as required. this could include staff from other professional backgrounds, including people from other organisations, patients , carers and relatives.
  • Mentor and facilitate the training and development of staff, both individually and as teams: This includes students of all disciplines who are based in the CRHT as well as post graduate CRHT members.

  • Create learning environment for students and forge links with education providers.

  • Oversee and participate in mandatory training programmes to ensure that CRHT staff comply with Health board Policies.

  • Maintains patient records to ensure information is accurate and up to date for future reference and complies with the Data Protection Act, schedules meetings to fit with other standing diary commitments.

  • Prepares care plan to ensure the delivery of effective patient care.

  • Consider the job in a time frame ie things done on a daily, weekly, monthly, annual or longer basis.

  • Physical dexterity, skills and accuracy to administer oral/intramuscular medication, perform venepuncture, venous cannulation, undertake physical diagnostic/investigative examinations and to monitor physical health using appropriate equipment e.g. stethoscope, sphygmomanometer.

  • Keyboard skills and information technology skills including word processing, email, intranet and internet, power point for communication, correspondence, clinical reports, teaching preparation, clinical literature searches.

  • Physical dexterity and fitness to undertake Restricted Physical Intervention training and interventions when required.

  • Physical fitness to escort patients inside and outside of the hospital setting.

  • The post holder will have financial responsibility and will need to manage the budget adequately.

  • The post holder will be required to authorise the use of bank to ensure the rota is covered working with your senior nurse to ensure that you also reach financial targets.

  • The post holder will be expected to monitor and verify travel expenses.

  • The post holder will be expected to order medical/ surgical, stock and non-stock.

  • Ensure the team submit timesheets in a timely manner following verification from the post holder.

  • To ensure the safe administration of medication and the correct policy is followed by all staff.

  • Be guided by broad occupational policies.

  • Have responsibility for the management of a defined group of staff within the inpatient service.

    The CRHTT is undergoing a period of redevelopment and redefinition within Cwm Taff Morgannwg and the team leader will be a key agent of change for a service that will have a focus on psycho social intervention within a community setting as an alternative to hospitalisation, alongside the requirement for assessment at times of crisis. The successful candidate will be an accomplished practitioner and have experience leading people and driving forward Person centered care. They will have values that reflect a compassionate and open minded professional who has the confidence and ability to provide pragmatic and evidence based alternatives to hospitalisation.


  • Applicants will need relevant experience of working in Community or Acute Mental Health Teams and will be comfortable with working with a complex and at times high risk client group, ensuring the safety of service users and Team members alike. The postholder will work comfortably within a Multi-Disciplinary Team and embrace the opportunities for learning that comes with that, but have the confidence and experience to work autonomously under challenging conditions. They will work with the Team and the Service User to plan and provide care in innovative ways, and be prepared to think creatively while supporting colleagues both within the Team and the wider Health community .
