Full Job Description

NHS Lincolnshire Integrated Care Board has been established to support the wider health and social care needs of the population of Lincolnshire and will focus on to deliver its agreed vision of 'Better Lives for the people of Lincolnshire'.
This includes:
+ Improving the health of children and young people.
+ Supporting people to stay well and independent.
+ Acting sooner to help those with preventable conditions.
+ Supporting those with long-term conditions or mental health issues.
+ Caring for those with multiple needs as populations age.
+ Getting the best from collective resources so people get care as quickly as possible., The post holder will work as a member of the NHS Lincolnshire Integrated Care Board Continuing Healthcare Team.
The post holder will support the CHC Business and Clinical heads of service in discharging their responsibility to provide assurance to all stakeholders that the CHC service are delivering a high quality, effective, person centred and value for money service.
The post holder will support the CHC team in the implementation of quality improvement and assurance programmes and will ensure that quality and performances of both internal and commissioned services through CHC are monitored effectively, enhancing the patient experience
The post holder will be required to develop and maintain effective communications with a range of people including clinicians in health and social care settings, LICB managers and directors, private providers, charities and commissioning support services, on complex matters in relation to quality and performance.
The role will support the delivery of the LICBs statutory obligations in respect of CHC delivery, as well as supporting Lincolnshire's Personal Health Budget programme and leading the delivery of the QIPP programme
The post holder will support the provision of effective quality work programmes, to enable Lincolnshire ICB to meet its statutory responsibilities, The post holder is responsible for implementing, undertaking and monitoring a range of quality and governance processes. This includes, but is not limited to:
Supporting the quality monitoring of care providers including care homes and domiciliary providers, responsible for completing quality visits and reporting findings to the quality lead
Responsible for continuing healthcare eligibility and care package sign off up to matrix authorisation level, ensuring package meets the needs of the person and offers value for money.
Supports the authorisation of fast-track funding within the 2-hour time frame
Supports the internal audit of CHC assessment documentation including PHB
Undertaking CHC training for internal and external staff and stakeholders
Responsible for completing retrospective reviews including completing needs portrayal documents, DSTs, meetings with the individuals and their family
Attends the CHC eligibility panel as a panel member
Conducts Local resolution meetings following Disputes
Supports Patient experience surveys & feedback
Where appropriate, support development of documentation, policies & procedures
Supporting the development of the monthly quality report
Supports freedom of information requests and subject access requests
Undertake DSTs and reviews if required, and to ensure clinical CHC knowledge is maintained
Adopt a matrix team working style working closely with CHC's and ICBs other key functions and teams

Physical skills

+ Elements of this role will require direct involvement with individuals and their representatives, via telephone or on-line platforms, and face to face
+ Support individuals and their representatives to contribute to and be confident in the CHC process by encouraging their participation in a transparent and equitable process.
+ Demonstrates ability to complete documentation or enter data accurately, consistently and in a timely manner
+ Ensures credible evidence is documented fully to support decision making and later review of the case through audit and/or independent review
+ The post requires skills of evaluating care. Accuracy is important. Good observational skills are essential
Policy and Service Development
+ Support the development and implementation of a range of policy and service developments that meets the needs of Lincolnshire in relation to the CHC service
+ Required to analyse, interpret, and communicate complex national directives into processes, policies and presentations for application.
+ Quality monitor services commissioned by the continuing health service
+ Monitor and analyse the patent experience
Financial and physical resources
+ Act in a way that is compliant with standing order and standing financial instructions in the discharge of this responsibility
+ Constantly strive for value for money and greater efficiency in the use of these budgets
+ Authorise expenditure as required within NHS Lincolnshire ICBs Standing Financial Instructions
Human Resources
+ If required, to line manage team members
+ Ensure own compliance with all statutory and mandatory training
+ Support training and induction of new staff
Information resources
+ All staff members who contribute to patients' health records or have access to patient/staff information are expected to be familiar with, and adhere to, GDPR and ICB Information Management Policies.
+ All staff who have access to patient/staff records/information have a responsibility to ensure that these are maintained efficiently, and that confidentiality is protected in line with the ICBs' Confidentiality & Disclosure Policy.
+ All staff members have an obligation to ensure that health records are maintained efficiently and that confidentiality is protected. Staff members are also subject to this obligation both on an implied basis and also on the basis that, on accepting their job description, they agree to maintain both patient / client and staff confidentiality. In addition, all health professionals are advised to compile records on the assumption that they are accessible to patients in line with GDPR 2018.
Research and Development
+ Undertake research and development as required to support the continued improvement of the service.
+ Deliver projects to comply with key performance indicators
Other commissioning duties as directed by Lead Commissioners
+ To represent the ICB and their interests, and CHC Leads, as required at local and national events
+ Possess extensive working knowledge of CHC to be able to provide specialist CHC advice
+ To undertake other duties commensurate with this band as and when requested
+ To keep updated as to current local and national policy and strategy
+ To keep updated as to improvement thinking, tools and techniques
+ Remain aware at all times of the links between this role and other functions of the CHC working collaboratively where required with clinical, quality, contracting and finance
+ To work with regard to the ICB's policies and procedures at all times.
+ Deliver against team objectives, achieving quality outcomes, prioritising own workload and working to tight deadline
