Full Job Description

Inpatient services provide comprehensive care to patients creating a comfortable environment for treatment. They have responsibility for assessment of individual patient care needs and development, implementation and evaluation of programmes of care.
The Rehabilitation Nursing Team care for patients aged 16 and above during their neurological rehabilitation journey, working as part of a specialist MDT to help achieve patient goals.
Early Intervention Community Team is an integrated service aiming to build relationships and break barriers between organisations, whilst working effectively for the person, avoiding duplication and reducing delays, empowering staff to be decision-makers and building trust with the wider system.
Community Teams work with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients to ensure their independence and wellbeing.
Learning Disabilities In Patients provide support to service users who have challenging behavioural needs, physical disabilities and additional health needs. They deliver short breaks and day services for people needing continuing health care in a more specialised environment, as well as providing residential care for people with complex health needs.
Learning Disabilities Community Teams provide healthcare for people with learning disabilities living in the community. The service aims to provide high quality care via multidisciplinary working and close collaboration with other agencies.
